Capitol Health Associates Inc. in New Jersey is managed under the support coordination process under the Division of Developmental Disabilities (DDD) and we are an approved provider in NJ. We have been qualified by DDD to design, approve, coordinate, and monitor a plan of support services that will assist you in discovering your potential and reaching your life goals. When you select us as your support coordination agency in NJ, our dedicated team goes above and beyond to create the perfect Individualized Support Plan (ISP) for you and your family and loved ones.

Overview of Support Coordination in New Jersey

All individuals who are eligible for and wish to access Division of Developmental Disabilities services must either choose or be assigned to an NJ Support Coordination Agency. The Support Coordination Agency acts as a guide in the world of the Division of Developmental Disabilities Services. From therapeutic horseback riding to cooking classes and game design, Capitol Health Associate’s DDD Support Coordination services will help you explore your interests while you learn new life skills for your present and your future.

Our Support Coordinators listen to your needs and goals in life and make sure to find service providers in the community that can meet your needs. We help to create an Individualized Service Plan and check in to make sure the services are meeting the individual’s needs. We are here to answer all your questions. We operate 7-days a week with a 24-hour emergency hotline to help you whenever you are in need.

Capitol Health Associate’s Support Coordinators are experienced professionals who will consult with you to locate programs and services such as:

  • Day Programs

  • Home Care Services
  • Transportation Providers
  • Recreation and Social activities
  • Community Inclusion Programs

Our support coordination services are designed to facilitate positive outcomes with our individuals by following a process that is centered around our individuals and their families. At Capitol Health Associates Inc., we utilize a Person-Centered approach that centers the voices of the individuals in designing their own services. We strive to recognize and create a bond with individuals and families so we can match them up with the services that will best serve their needs.

  • We determine the individual’s support needs with their participation in the IDT process.
  • We work with the individual and their family to develop the NJ Person-Centered Planning Tool (PCPT) and NJ Individual Service Plan (NJISP).
  • We work with service providers to provide appropriate services for the individual based on funds available in the individual’s budget.
  • We continuously monitor the individual’s support and services.
  • We are available to respond to emergencies and other service-related needs of the individual and/or family.

Our support coordinators strive to establish a bond with the individual and their families to truly understand each individual’s unique situation. This allows us to coordinate the services that will best serve their specific needs.

How to Choose Capitol Health Associates Inc. as Your Support Coordination Agency

Select Capitol Health Associates Inc. on your Support Coordination Agency Selection Form and follow the instructions to submit it to DDD.

DDD Eligibility Requirements in New Jersey

An individual must be 18 or older to be evaluated by DDD for functional eligibility for developmental disability services. An individual must be 21 or older to access DDD services.

How Our Support Coordination Services Work

Any individual who is eligible for and wishes to access DDD services in New Jersey may choose Capitol Health Associates Inc. as a Support Coordination Agency. Our Support Coordinators live around New Jersey and are knowledgeable about services available in your community. We also have French and  Spanish-speaking Support Coordinators. Most services to individuals with developmental disabilities are delivered at home and in community-based settings.
